Common Issues When Finalizing EMRs

Patient Address Not Found (for facility patients)

To finalize a Plan of Care (POC), the patient must have an address on file that covers the certification period, as this address is printed on the document.

For facility patients, we use the facility’s address.

The issue arises when the patient’s facility admission date falls after the start of the certification period.

To resolve this, the facility admission date should be updated to either the start date of the certification period or an earlier date.
